The Integrated Team Care (ITC) Program aims to: 

 

Contribute to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through:

• Better access to coordination 

• Better access to multidisciplinary care

• Support for self-management

Improve access to culturally appropriate mainstream primary care services for Aboriginal and Torres Strait Islander people

• General practice

• Allied health and specialists

• Pharmacy

• Other Healthcare services

If you are eligible and feel you could benefit from the ITC Program, you can ask your doctor for a referral.

Referrals are accepted from both ATSICHS Mackay and private general practices.
